Karli Morgenthau's death. He tried so hard to reach her, to bring her in from the cold, and he got so close to succeeding. She was misguided teenager who fought for a good cause, and he wishes he could have done right by her and saved her. He wants to be the kind of hero someone like Karli needs and wants to see in the world - so failing her sits harder on him than any other past regrets and failures that weigh on him, though it's just the latest in a chain of trauma related to failing to save people.
